Paul Marik, MD, one of the most highly published critical care physicians in the world and the Director of the ICU at Sentara Norfolk General Hospital, was recently told by Sentara Healthcare that he could no longer administer a range of highly effective COVID-19 treatments to critically ill patients - the same treatments he has successfully used to reduce COVID deaths in the ICU by as much as 50%. The result of the prohibition has been a sharp increase in patient mortality. Because Dr. Marik can no longer stand by while patients needlessly die without proper treatment, he has filed a lawsuit to allow him and his colleagues to administer the combination of FDA-approved drugs and other therapies that has saved thousands of critically ill COVID-19 patients in the last 18 months. The Complaint filed today in the Circuit Court for the City of Norfolk, Virginia states that Sentara Healthcare is “preventing terminally ill COVID patients from exercising their right to choose and to receive safe, potentially life-saving treatment determined to be appropriate for them by their attending physician.” Under Virginia law, every patient has the right to receive treatment deemed appropriate for them by their attending physician, and terminally ill patients have the right to try investigational medicines that their treating physician recommends. Through its arbitrary prohibition of the COVID-19 treatment protocol ... Sentara is violating the law and unjustly depriving critically ill patients of lifesaving treatment.

Does the FDA think these data justify the first full approval of a covid-19 vaccine?

On 28 July 2021, Pfizer and BioNTech posted updated results for their ongoing phase 3 covid-19 vaccine trial. The preprint came almost a year to the day after the historical trial commenced, and nearly four months since the companies announced vaccine efficacy estimates “up to six months.”. But you won’t find 10 month follow-up data here. While the preprint is new, the results it contains aren’t particularly up to date. Since late last year, we’ve heard that Pfizer and Moderna’s vaccines are “95% effective.” Measuring vaccine efficacy two months after dosing says little about just how long vaccine-induced immunity will last. “Waning immunity” is a known problem for influenza vaccines, with some studies showing near zero effectiveness after just three months. And so the recent reports from Israel’s Ministry of Health caught my eye. In early July, they reported that efficacy against infection and symptomatic disease “fell to 64%.” By late July it had fallen to 39% where Delta is the dominant strain. This is very low. For context, the FDA’s expectation is of “at least 50%” efficacy for any approvable vaccine. Now Israel, which almost exclusively used Pfizer vaccine, has begun administering a third “booster” dose to all adults over 40. The US plans to follow suit. Until new clinical trials demonstrate that boosters increase efficacy above 50%, without increasing serious adverse events, it is unclear whether the 2-dose series would even meet the FDA’s approval standard at six or nine months.

Israel offered the world a hopeful glimpse of the way out of the pandemic. No longer. A fourth wave of infections is rapidly approaching the levels of Israel's worst days of the pandemic last winter. The daily rate of confirmed new virus cases has more than doubled in the last two weeks, making Israel a rising hot spot. Restrictions on gatherings and commercial and entertainment venues were reinstated this week, and the government is considering a new lockdown. Some experts fear that Israel's high rate of infections among early vaccine recipients may indicate a waning of the vaccine's protections over time. The vaccine may be less effective at preventing infection with the highly contagious Delta variant. The vast majority of Israel's older population had received two doses of the Pfizer-BioNTech vaccine by the end of February, and by now about 78 percent of the population 12 and older are fully vaccinated. Data published by Israel's Ministry of Health in late July suggested that the Pfizer shot was just 39 percent effective against preventing infection in the country in late June and early July, compared with 95 percent from January to early April.

The larger the foundation became, the less anyone seemed willing to ask tough questions about its secretive management structure or its penchant for giving money to lucrative pharmaceutical and credit card companies such as Mastercard, despite the fact that giving away billions to wealthy corporations set an unusual and troubling precedent in the philanthropic sector. Billionaires who make their fortunes through corporate practices that undercut workers and deepen inequality — like corporate tax avoidance, insufficient sick pay and the immoral gap in pay between executives and low-paid workers — are not the solution to problems they generate. In April last year, the University of Oxford was reportedly considering offering a Covid-19 vaccine developed by its scientists on a nonexclusive basis. But then, Kaiser Health News reported, “Oxford — urged on by the Bill & Melinda Gates Foundation — reversed course. Half of world’s population worse off due to the pandemic, survey finds

This translated to 1.6 billion adults globally, it said. “Worldwide, these percentages ranged from a high of 76 per cent in Thailand to a low of 10 per cent in Switzerland,” said researchers in a statement. In Bolivia, Myanmar, Kenya, Uganda, Indonesia, Honduras and Ecuador, more than 70 per cent of people polled said they took home less than before the global health crisis. In the United States, this figure dropped to 34 per cent. The Covid-19 crisis has affected workers across the world – particularly women. International charity Oxfam said ... that according to its own research, the pandemic had cost women around the world $800bn (£578bn) in lost income. The poll also showed that one in three people surveyed had lost their job or business due to the pandemic – translating to just over 1 billion people globally. When Covid cases were rising in the U.S., the news coverage emphasized the increase. When cases were falling, the coverage instead focused on those places where cases were rising. And when vaccine research began showing positive results, the coverage downplayed it, as far as [Dartmouth professor Bruce] Sacerdote could tell. He began working with two other researchers, building a database of Covid coverage from every major network. The researchers then analyzed it with a social-science technique that classifies language as positive, neutral or negative. The results showed that Sacerdote’s instinct had been right. The coverage by U.S. publications with a national audience has been much more negative than coverage by any other source that the researchers analyzed, including scientific journals, major international publications and regional U.S. media. About 87 percent of Covid coverage in national U.S. media last year was negative. The share was 51 percent in international media, 53 percent in U.S. regional media and 64 percent in scientific journals. Notably, the coverage was negative in both U.S. media outlets with liberal audiences (like MSNBC) and those with conservative audiences (like Fox News). If we’re constantly telling a negative story, we are not giving our audience the most accurate portrait of reality. As Ranjan Sehgal, another co-author, told me, “The media is painting a picture that is a little bit different from what the scientists are saying.”

That's the equivalent of 255 million full-time jobs lost across the globe, or about four times greater than the impact from the Great Recession of 2009. The world's poorest could take a decade to regain their financial footing. Oxfam describes the pandemic's impact as "the greatest rise in inequality since records began." The International Labour Organization said the crisis has been the most severe on work since the Great Depression in the 1930s. "Its impact is far greater than that of the global financial crisis of 2009," said ILO Director-General Guy Ryder. America's richest people have seen their wealth soar during the pandemic by more than $1 trillion, thanks to a booming stock market and a K-shaped recovery that has benefited the rich, while poorer people have struggled with lost wages and jobs and future opportunities. When I called up Chuck Collins on Tuesday afternoon, I found him glued to one of the grimmest new metrics documenting America’s economic and social unraveling. Collins is a scholar of inequality at the Institute for Policy Studies, a progressive think tank, and since March he has been tracking how the collective wealth of American billionaires has been affected by the coronavirus pandemic. In previous recessions, Collins said, billionaires were hit along with the rest of us; it took almost three years for Forbes’s 400 richest people to recover losses incurred in 2008’s Great Recession. But in the coronavirus recession of 2020, most billionaires have not lost their shirts. Instead, they’ve put on bejeweled overcoats and gloves made of spun gold — that is, they’ve gotten richer than ever before. On Tuesday, as the stock market soared to a record, Collins was watching the billionaires cross a depressing threshold: $1 trillion. That is the amount of new wealth American billionaires have amassed since March, at the start of the devastating lockdowns that state and local governments imposed to curb the pandemic. On March 18, according to a report Collins and his colleagues published last week, America’s 614 billionaires were worth a combined $2.95 trillion. When the markets closed on Tuesday, there were 650 billionaires and their combined wealth was now close to $4 trillion. Half of CDC Coronavirus Test Kits Are Inaccurate, Study Finds

According to a July 17 study published in the International Journal of Geriatrics and Rehabilitation, 50 percent of nucleic acid coronavirus tests distributed by the Centers for Disease Control and Prevention (CDC) provided inaccurate results. The study's lead author, Sin Hang Lee, MD, director of Milford Molecular Diagnostics Laboratory, found that the testing kits gave a 30 percent false-positive rate and a 20 percent false-negative rate. To determine these false-positive and false-negative rates, the Connecticut State Department of Public Health Microbiology Laboratory provided Lee 20 tests, which were then re-tested using his own methodology, which examines samples on a cellular level, rather than just testing fluid with no cellular matter from potentially infected oral and nasal secretions. While the results of Lee's testing may be alarming, they also pointed to yet another discovery: new mutations of the virus. Two tests that initially provided false-negatives and one test that yielded a positive result were actually found to be positive for coronavirus and a mutation of the virus, meaning two variants of the virus can simultaneously infect one person. However, it's not just the test you use that may be contributing to inaccurate resultswhen you get tested is important, too. Getting tested the day a person is infected with coronavirus will likely yield a 100 percent false-negative rate; by day 8 after becoming infected, however, that rate drops to just 20 percent.

Gilead's $2,340 price for coronavirus drug draws criticism

The maker of a drug shown to shorten recovery time for severely ill COVID-19 patients says it will charge $2,340 for a typical treatment course for people covered by government health programs in the United States and other developed countries. Gilead Sciences announced the price Monday for remdesivir, and said the price would be $3,120 for patients with private insurance. The amount that patients pay out of pocket depends on insurance, income and other factors. The price was swiftly criticized; a consumer group called it an outrage because of the amount taxpayers invested toward the drug's development. In 127 poor or middle-income countries, Gilead is allowing generic makers to supply the drug; two countries are doing that for around $600 per treatment course. The drug, given through an IV, interferes with the coronaviruss ability to copy its genetic material. In a U.S. government-led study, remdesivir shortened recovery time by 31% 11 days on average versus 15 days for those given just usual care. Peter Maybarduk, a lawyer at the consumer group Public Citizen, called the price an outrage. Remdesivir should be in the public domain because the drug received at least $70 million in public funding toward its development, he said. The price puts to rest any notion that drug companies will do the right thing because it is a pandemic, Dr. Peter Bach, a health policy expert ... said. The price might have been fine if the company had demonstrated that the treatment saved lives. It didnt.

As Europe emerges from lockdown, the question hangs: was Sweden right?

As Europe and the world emerge blearily to survey the wreckage of lockdown, the question is still left hanging. Was Sweden right? Stockholm gambled in its response to coronavirus, but neither its economy nor its healthcare system have collapsed. Just two months ago, it held hands with Britain in rejecting total lockdown. Then on 23 March, Boris Johnson did a U-turn, leaving Sweden ... on its own. Since then the divergence has become radical and political. The one table that glares at us daily is ... deaths per million. The most stringent lockdowns as in China, Italy, Spain, New Zealand and Britain have yielded both high and low deaths per million. Hi-tech has apparently worked in South Korea, but so has no-tech in Sweden. Swedens 319 deaths per million is far ahead of locked-down Norways 40 and Denmarks 91, but its well behind locked-down UKs 465 and Spains 569. Swedens light-touch policy is led by two scientists, Johan Giesecke and his protg Anders Tegnell. The latter currently leads Stockholms strategy with ... 73% popular support. Tegnell has been emphatic throughout. A degree of social distancing and avoiding crowds is enough. As for lockdown, Nothing to do with [it] has a scientific basis. [Sweden] has kept itself open and at work, and has not seen the surge in all-causes excess deaths of the UK and other high-lockdown states. According to Tegnell ... there is no other escape but to find ways of living with this virus. Sweden gambled in its response, but so did the rest of the world. The UN warns that the world could lose four years of growth at a cost of $8.5 trillion. Famine and further disease will be rife. That was surely the greater gamble.

Lately, my country has caught the attention of the media. Swedens response to the pandemic has been singled out as radical, lax and controversial because Sweden has not imposed a broad general lockdown. Sweden is known as a country with a strong welfare model, including public healthcare for all, and has among the worlds highest life expectancies. Some might find it difficult to reconcile this image with our approach to containing COVID-19. Sweden shares the same goals as all other countries to save lives and protect public health. Swedens measures differ from other countries in a few significant ways. We are not shutting down schools for younger children or daycare facilities. We have no regulation that forces citizens to remain in their homes. And we have not ordered the closure of any businesses. Swedish laws on communicable diseases are mostly based on voluntary measures and on individual responsibility. The use of recommendations in public health efforts rather than mandates is a common strategy in Sweden. One example of this is child immunizations. In contrast to the United States, where all 50 states mandate immunizations for children in order to enroll in school ... Swedens child vaccination program is based on recommendations from the authorities and is not a legal requirement. Yet more than 97% of Swedish children are vaccinated. Swedens strategy may not provide all the answers, but we believe the combination of voluntary and mandated measures is not only more sustainable for Sweden than a lockdown strategy but will strengthen the resilience of Swedish society to fight this virus in the long run.

Over 43,000 US millionaires will get stimulus averaging $1.6 million each

At least 43,000 American millionaires who are too rich to get coronavirus stimulus checks are getting a far bigger boost averaging $1.6 million each, according to a congressional committee. The Coronavirus Aid, Relief, and Economic Security (CARES) Act trumpeted its assistance for working families and small businesses, but it apparently contains an even bigger benefit for wealthy business owners, the committee found. The act allows pass-through businesses ones taxed under individual income, rather than corporate an unlimited amount of deductions against their non-business income, such as capital gains. They can also use losses to avoid paying taxes in other years. That gives the roughly 43,000 individual tax filers who make at least $1 million a year a savings of $70.3 billion or about $1.6 million apiece, according to the Joint Committee on Taxation. Hedge-fund investors and real estate business owners are far and away the ones who will benefit the most, tax expert Steve Rosenthal [said]. Rep. Lloyd Doggett (D-Texas) claimed that someone wrongly seized on this health emergency to reward ultrarich beneficiaries. For those earning $1 million annually, a tax break buried in the recent coronavirus relief legislation is so generous that its total cost is more than total new funding for all hospitals in America and more than the total provided to all state and local governments, he stressed in a statement.

Across the country, more than 9,200 health workers out of an estimated 18 million have the disease, the CDC reported Tuesday in a separate paper, which notes that the number probably understates the true number of coronavirus infections. Dr. Robert Siegel, an infectious disease expert at Stanford, called the relatively low infection rate from the Solano County patient a hopeful sign. But he added that public health officials should remain vigilant in protecting workers. The results are promising for health care workers. E.P.A., Citing Coronavirus, Drastically Relaxes Rules for Polluters

The Environmental Protection Agency on Thursday announced a sweeping relaxation of environmental rules in response to the coronavirus pandemic, allowing power plants, factories and other facilities to determine for themselves if they are able to meet legal requirements on reporting air and water pollution. The move comes amid an influx of requests from businesses for a relaxation of regulations as they face layoffs, personnel restrictions and other problems related to the coronavirus outbreak. Issued by the E.P.A.s top compliance official, Susan P. Bodine, the policy sets new guidelines for companies to monitor themselves for an undetermined period of time during the outbreak and says that the agency will not issue fines for violations of certain air, water and hazardous-waste-reporting requirements. Companies are normally required to report when their factories discharge certain levels of pollution. The order asks companies to act responsibly if they cannot ... monitor or report the release of hazardous air pollution. Businesses, it said, should minimize the effects and duration of any noncompliance and keep records to report to the agency how Covid-19 restrictions prevented them from meeting pollution rules. Gina McCarthy, who led the E.P.A. under the Obama administration ... called it an open license to pollute. She said that while individual companies might need flexibility, this brazen directive is nothing short of an abject abdication of the E.P.A. mission to protect our well being.

My adulthood has been punctuated by severe national emergencies. The first my generation experienced was the terrorist attack on September 11, 2001. The government quickly responded by attempting to achieve two things: one, expanding executive power, and two, transferring public wealth into private corporations. The second national emergency my generation experienced was the 2008 housing bubble collapse and subsequent recession. Again the federal government ... sought to exploit the crisis to move vast wealth from the public treasury into private bank accounts. A staggering $14tn was transferred from taxpayers to private hands. [The] latest iteration is the Covid-19 pandemic. Once again, the federal government appears poised to exploit this emergency to expand executive power and move wealth from the public treasury into private bank accounts. As we witnessed with the authoritarian reactions to 9/11, emergency violations of civil liberties are not easily rolled back, and often aggregate over time. In the wake of 9/11, Congress passed the National Defense Authorization Act, which gave sweeping powers to the executive branch. In 2012, Obama signed an expanded version into law, which gave the president the power to hold individuals, including US citizens, in military detention indefinitely, which means for life. We must reject such authoritarian measures wholly.

Scientists at the center of the “lab leak” controversy visited Anthony Fauci’s institute at the National Institutes of Health in 2017 to discuss their research — just months before NIH lifted a pause on high-risk virology. Wuhan Institute of Virology Senior Scientist Shi Zhengli [visited] National Institute of Allergy and Infectious Diseases staffers in June 2017, where she gave a presentation about novel coronaviruses. Shi is known internationally as the “Bat Lady” for her work with bats and their coronaviruses. Though ostensibly a civilian lab, the Wuhan Institute of Virology has conducted research projects “for defensive and biosecurity needs of the military” since at least 2017, according to U.S. intelligence. EcoHealth Alliance, a U.S. research organization, partnered closely with the Wuhan Institute of Virology, subcontracting NIAID funding to Shi’s lab, and arranged the meeting. Fauci himself met with [EcoHealth Alliance President Peter] Daszak four months later, in October 2017. By December 2017, NIH had resumed funding for gain-of-function research that generates new viruses in the lab following a three year pause and debate about the possibility that such research could cause a pandemic. A 2021 State Department fact sheet stated that “despite the WIV presenting itself as a civilian institution,” it has conducted classified research on behalf of the Chinese military “since at least 2017.”

During the entirety of the ongoing two-and-a-half-year public health emergency, while he was acting as the president’s top COVID-19 advisor, Dr. Fauci made no effort to genuinely investigate COVID-19’s origins. On May 11, 2021, Senator Rand Paul finally asked: “Dr. Fauci, do you still support NIH funding of the lab in Wuhan?” Dr. Fauci angrily denounced the question: “Senator Paul, with all due respect, you are entirely, entirely, and completely incorrect. That the NIH has not ever and does not now fund gain-of-function research in the Wuhan Institute of Virology.” Dr. Fauci was then under oath, so his blanket denial was a perjury of striking audacity: [The National Institute of Allergy and Infectious Diseases'] decade-long funding of gain-of-function experiments at the Wuhan lab had left a public record abundantly and unambiguously documented on PubMed, the official NIH archives of the world’s peer-reviewed published research. The authors of myriad gain-of-function studies openly thank NIAID and NIH for funding their research at the Wuhan lab. The ease of finding these incontrovertible proofs of his deception makes Dr. Fauci’s lie seem reckless. But the savvy NIAID chief evidently calculated that the issue was now so politicized and the media so committed to fortifying official government orthodoxies that truth was irrelevant. The nation’s leading journalistic outlets abetted Dr. Fauci’s public deception by shielding him from difficult questions.
